Replace `INPUT_NAME` with `INPUT_LABEL`

Not sure how to explain this but simply if you look at the screenshot below:

You can see how they all say Input which can get confusing so I am recommending showing the input label instead of the input type, or maybe some way of showing both by separating them by the | symbol or even doing LABEL (TYPE)
